Making changes to your account is easy and Home Federal maintains the security of your information when making changes to your account. We realize your time is valuable and want to make sure the time you spend making changes to your account is minimal, again, while providing peace of mind knowing your information is secure. Depending on the request, either call or simply print off the appropriate form below, sign and send to Home Federal, so we may accommodate your request on your account.
Name Change
Call our customer service department at 1-(800) 888-7283, Monday through Friday, 8:30 a.m. until 6:00 p.m. and Saturday, 9:00 am until 5:00 pm MST, they will request the appropriate information, typically, a marriage certificate and provide you either a fax number (208) 468-5045 (please notify us prior to faxing), or mailing instructions depending on your requested method.

Address Change

Designation of Beneficiary
At account opening you had the opportunity to provide us with both primary and secondary beneficiary (ies). If you selected and are satisfied with your appointed beneficiary (ies), there is nothing else for you to do. If you are making any of the following changes listed below. Please Click Here.
Initial Beneficiary Designation: I did not designate the beneficiary(ies) of my HSA at account opening and need to do so.
Replace Beneficiary(ies): I need to replace the individual(s) named in my initial designation of the account, by doing it will revoke all prior beneficiary(ies) designations.
Add beneficiary(ies): I need to add individual(s) named in my initial designation my HSA account. This list supplements, but does not replace, the beneficiary(ies) previously designated by me on the date specified. Please note, when adding beneficiary (ies), if the share % of previously designated beneficiary(ies) changes, restate all beneficiary(ies) and the corresponding share % if the previous percentages are no longer correct.
