The Health Savings Account and tax advantages associated with the account are based on Federal law, therefore, all of the forms referenced can be found at the official site of the Internal Revenue Service. www.irs.gov.

In addition to monthly bank statements, Home Federal provides each individual account holder with annual reporting of Contributions with IRS form 5498 -- SA and annual reporting of Distributions with IRS Form 1099 –SA. Let us take a closer look.

Contributions: IRS Form 5498-SA.
Home Federal issue date: January 31st of each tax year.

Please Note: You can make contributions to your Health Savings Account for a tax year until April 15th of the following year. For example, you can make a 2005 contribution to your HSA between January 1st, 2005 through April 15, 2006. For these account holders Home Federal will also provide a 5498-SA in May. You will only receive an additional 5498-SA in May if you make contributions between January 1st through April 15th for the prior tax year contributions.
